Abstract : Periapical tissue of 14 cases with chronic periapical lesions were studied bacteriologically, histologically and immunohistochemically to detect the possible improvement resulting from non-surgical treatment of such cases. They were divided into two groups according to the line. of treatment adopted. In the first group, periapi- cal surgery was performed without any prior treatment. In the second group, periapical surgery was performed one month after root canal treatment, non- surgical endodontic treatment of periapical lesions showed marked improvement based by the decreased bacterial count, the repair potential of the tissue and the decrease in T helper inducer cells with the increase in T suppressor/cytotoxic cells which denote the shift to down regulation of the immune response.
